Thank you for completing your previous survey submission. As a reminder, Survey (77) 2026 has been open since January and is available for real-time data entry, helping keep patient records accurate and current throughout the year.

Please continue to record all relevant events as they occur, including:

• New Patients
• New transplants
• Graft failure
• Transfers and changes in treatment
• Changes in home postcode
• Comorbidities
• Supplementary surveys, where applicable (Cancer, Parenthood, Rejection, HD-access related bloodstream infections, and PD-related infections)

Transplant patient anniversary follow-up data can be completed once the anniversary date is reached. A system report is available to help identify patients due for follow-up. If you require assistance or would like more information, please contact the Registry.
